Fees & Insurance

We are covered by all major insurance carriers including Medicare and Medicaid. Please check your insurance company's provider directory to see if your physician is listed.

If your insurance carrier requires referral forms, please make sure you get the necessary forms from your primary care or other referring physician before your visit.

For insured patients, we request payment of the insurance deductible only. Please make co-payments at the time the service is rendered. If this is not possible, please notify our Patient Accounts Representative to work with you to develop a payment plan that fits your budget.

You may receive a bill from us for the balance owed after your insurance company makes its payment. This balance will be due upon receipt. If you need to make payment arrangements, please contact our Billing Department ahead of time.

For your convenience, we accept Visa, MasterCard, checks and cash.

As a service to you, our office staff will complete and submit the required claims to your insurance carrier for all of your fees.

If you have any questions regarding fees or your insurance coverage, please call our Billing Department. We are always available and happy to discuss your account with you.

Remember, your insurance policy is a contract between you and your insurance company. You are financially responsible to Jordan & Associates Gastroenterology for non-payment of any fees not covered by your insurance.
